elevated CPKs

I am a 40 y/o male.  My CPKs have been elevated for the past 3 years (possibly longer.  they were just monitored starting 3 years ago).  The ranges are between 200-500.  I have been very active in the past, strength training, aerobic exercise, sports, etc., so I have a bit more lean mass.  My testosterone is abnormally low, for which I take IM injections, 1/2 dose weekly vs. 1 dose every other week.  I am sleepy all of the time, often needing to stop on the side of the road for a nap on a normal 45 minute trip.  Sometimes I am so tired, it seems as though it would take all of my energy to simply get off the couch and walk across the room.  My TSH is borderline low, 0.95 with the lower limit of normal being 0.55 (i think).  My eosiniphils were slightly increased, by 0.2 %, recently for which i attribute my seasonal allergies.  I am 5'11" and weigh 185.  I have been diagnosed with sleep apnea and where a CPAP at night.   I was misdiagnosed with MS in 1998 based solely on an abnormal MRI (sporadic demyelination which has been stable since the initial MRI).  I took interferon A weekly IM injections for 6 years before it was discovered I was misdiagnosed.  I asked the neurologist if it wasn't MS, what it was.  He said that if I was lucky, I would never know what it was.  And to top it all off, I have ED (possibly stress induced maybe med induced).  I have experienced several stressors over the past 2 years, loss of a job, marriage dissolution, returning to college, and raising my four girls with 50% custody.  I had been on Lexapro 10 mg QD for about 8 years with no ED problems.  I switched to effexor 150 mg QD at about the same time as my stressors began.  Recently, my arms and shoulders have began to ache occasionally.  I take naproxen which alleviates the ache.  I lack the ability to concentrate and have a poor memory, both long and short term., but not to the point that it affects my daily activities.
