simple way that I can understand

At the L4-5 level, there is a small diffuse disc bulge measuring approximately 2 mm, with a superimposed right paracentral disc protrusion measuring approximately 6 mm AP. proplapsing superiorly by 3-4 mm. There may be minimal 2 mm anterolisthesis, degenerative in nature, secondary to severe facet arthropathy. There is no significant central canal narrowing. There is moderate to severe foraminal on the right with encroachment on the right L4 nerve, and mild-to-moderate foraminal narrowing on the left

The orthopedic surgeon or neurosurgeon can explain this.  Basically there are degenerative changes of aging and some of the changes are causing pressure on nerves.  A disc is bulging .  You can look up individual word meanings on the internet.  In Google, type things like, what is anterolisthesis?
