how to best id spinal nervesnerves

hello my daughter had tcs,tight filum terminal with a lipona in the filum,and a dermal sinus, her nuerosurgeon initially told us that she had a nerve root stuck but did ot include this in his operative report which was dictated 2 wks after her surgery,  so how would be the best way to find out if there is a nerver root stuck in her lipoma? also how would be the best approch to getting this resolved surgically he did very well i am not upset over that but her record in general in mixed up she was seen by about 7 NS residents and every visit started with know what did she have done there are also discrefancies in her Discgarge Summery, it states katelynn had no headache, N|V, or fever.   and katelynn had all the above.  any ideas please it is important this is cleared up for her sake and before i speak with her PCP i would like to know better how to handle this and knot upset any one in the process.

1.  how to best identify nerve root stuck i lipoma?
2. How to best handle the dr so none of them get upset?

thanks very much i appologize but this is very fustrating snd ss you know if it is not documentef it did not happen so please help.

An MRI of the lumbar spine would show the lipoma and should show a spinal root if there is one present in the lipoma, but it may be difficult to tell if it is "stuck" or ensheathed in the lipoma.  The latter is more likely.  You can request the MRI, if one was done after surgery and have it read by another neuroradiologist.  The discharge summary is usually dictated by the resident on discharge and likely is not going to be changed as it was signed by the attending physician (in this case the surgeon) and now finalized.  But in the chart record itself, it will show what happened on a daily basis and will indicate fever, nausea, vomiting, medications given, etc.  You can request a copy of the chart.

thank you for answering my question the thing is she had a mri done before the surgery and it did not indicate the nerve being there it was a surprise would a countrast agent help better identify any possible nerve that may be pressent, also my daughter had her 1wk and 6wk follow up and doeas not go back for a year shouldent we go and make shure the syrnix is clearing up?
