how to recover from OCD second attack

I am 29 years old. 10yrs before, i was diagnosed as OCD. my ocd is related with my studies.i used to remember all things i studied. Confusion, can not understand the core meaning in studies, etc. i was serverly affected and depressed a lot. After one week of hospitalization and taking medication [Fluoxetine 20, Clomipramine 25]. some how i managed and made my mind peacefully and achieved in my academic. Four years later, i was too much involved in academic and energized with thinking and understanding scientific principles, stuffs like that. i went to an excited state. During that period,suddenly my mind switched to the old state. After this incident, once again i struggled to understand the concepts. feel very uncomfortable to learn new things. Again 4 years i managed with this struggle but i didnt recover fully. during this period i have completed my master study.my medications are still continuing. My problem now is, for the past 2 years, my mind is in completely relaxed state.never caring about anything. simply keep ideal and enjoy what i already learnt.never tried for learning new things.not lack of new ideas, completely zero ideas. cannot solve even simple arithmetic.i have never experienced like this before. for this state, i prefer better to stay in a state of 10 years before level.unfortunately, now i am doing research degree. I am feeling that i can not complete my PhD. can anyone suggest me how to overcome from this or diagonise the disease?. please help me...

Sounds like the past 10 years have been very difficult for you.  Has any raised the possibility of Bipolar disorder with you in the past?  The way that you describe switching from one state to another sounds very Bipolar (previously known as Manic-Depressive).   One state you describe is characterized by depressed mood, apathy, lack of motivation with difficulty thinking and the other as motivated, energized with improved cognition.  What you describe does not seem to fall very much along the lines of OCD.  Perhaps they just didn't come out in your desription, but a main feature of OCD is to have repetitive thoughts which are troubling, that may be temporarily alleviated by a particular activity, which is the compulsion part of OCD.  There is not really a major shift from one state into another with OCD, that the severity can wax and wane.  

I would definitely recommend discussing this with your mental health provider.  Hope this helps.  
PS. Don't be scared by the term Bipolar.  There are many new exciting treatments, and there is much more acceptance for the diagnosis these days.

You seem to suffering from a severe anxiety disorder. I would suggest you to seek professional help in the form of counseling.
The treatment of OCD includes psychotherapy and medications. Psycotherapy includes two popular techniques systematic desensitization and flooding.
Medications include tricyclic antidepressants like cloimipramine. Please consult a psychotherapist at your earliest. Take care!

Thanks a lot for both of your suggestions. I face severe headache(right sided) when i am forced to think anything academically. it happens particularly in one blood vessel (3 inch back of ear) i observed an increased blood flow.my head feels heavy pain and relaxing will take minimum  45 minutes. during the head ache period, i got confustion,frustration,worries,etc.. it happens only in the right side.not in the left side.I am a left handed person by birth but i write and eat in right hand. [Fluoxetine 20, Clomipramine 25] are the tablets i am taking for the past 10 years. I feel that my psychological disorder has some neurotic failure.. can u guide me the reason for this and how to cure this...
