Before we can ascribe the pain to the testicles or related structures, any spinal issues have to be ruled out. The spinal muscles are tense when one masturbates (but not always). Please consider visiting a neurologist if you feel you have any lower back problems.
There could be a varicocele or inflammation of the testicles. These are remote possibilities, but should be explored. I would suggest a visit to the urologist's office.
