I will take your two questions in order :
(1)MG is an immune mediated disease, caused by antibodies which circulate in the
  blood and affect the junction between nerve and muscle, as such it does nor remain
  localized to one side of the body, since blood flow, which carries the antibodies is not
  restricted to one side of the body.
  The symptoms might be noticed first in one area but a this stage if you had MG it would be
  generalized.
(2)Tremor is not a feature MG, although weak muscles can sometimes seem to tremble
   simply due to the effort needed to maintain a posture.
   if this is a true tremor it stongly suggests that your underlying
   problem is not MG.
Without the opportunity to examine you and review your test results
it is difficult to contribute more at this point,however if you wish to
