migraine in child

My son was diagnosed with migraines at 8 Yrs old
MRI normal.  He was put on periacton and that worked
but we had to keep increasing the dose until it was no
longer effective.  Now he is 10 and is on Elavil
50mg  daily.  He is still getting headaches 1 to 2 a
week, but Tylenol helps those.  Questions :
Does this sound like appropriate treament?
Is this a large dose of Elavil (his neurologist says
I can go up to 75mg)?
Are there any dangerous side effects?
Are there other options I should explore?
How about St. Johns Wort?
Thank-you!
