Chronic motor tic disorder. It is important to remember that in younger children, for example at the age of 3, certain mistakes in language articulation are
normalNormal saline flush/expected, and improve with age. The mistakes your brother makes may be normal for him, depending on his age. Some speech problems are normal up to the age of 5. However, they may also be due to an underlying problem. The problems related to speech could be local, due to problems in moving the tongue, lips, etc. from a neurologic causes (nerve or muscle), from central nervous system problems (such as apraxia of speech) or ENT causes, such as impaired hearing.
My advice is that you raise your brother's speech problem with his pediatrician. He may also benefit from evaluation by a certified speech pathologist. He/she can evaluate your brother's speech and determine whether or not this is normal or whether further evaluation is necessary. Referral to a neurologist may or may not be warranted depending on their assessment.
