My daughter (an adult) has had a serious problem with nausea and
vomiting for about 8 weeks.  She has been hospitalized for several
days for dehydration.  She has had a great variety of tests, including
an MRI, CAT scan, endoscopy, colonoscopy, blood tests, etc. and no one
has been able to figure out the cause.  She has discovered that if she
stays completely still she is all right, but as soon as she moves
around, she vomits. An ENT found nothing wrong. Because she has had
problems with depression in the past, the doctor thinks there may be a
psychiatric involvement, but I wonder if that is just a diagnosis by
elimination, since movement causing the problem seems to indicate a
physical cause.  Any help would be greatly appreciated. Thank you.

Hello, Elsie,
Your daughter's persistent nausea and vomiting is a very serious condition.  A lot of medical complications may appear if it is not treated aggressively.  From an etiological (causes) standpoint, this can be either central or peripheral.  The central causes in an young adult may include tumor, vascular malformation, and trauma.  The peripheral ones would encompass gastrointestinal abnormalities, vestibular and ENT problems.  Apparently, her doctors have thought about all of these possibilities and ordered a quite extensive work-up, which has been negative.  From an academic point of view, in addition to the studies that have been done, You may want to consider MRI with gad (contrast) to look for tumor, vestibular testing to check her balance system, MRA or cerebral angiogram to shoot at vascular malformation, and LP (spinal fluid exam) to rule out infection.  After all these, one possibility that can not be ignored is a stress related situation, which will require a full psychological and psychiatric evaluation.  Please feel free to visit our foundation where an evaluation for your daughter's condition can be done timely and comprehensively.  Call toll free (800) 223-2273 ask ext. 45559 or (216) 444-5559 for an appointment.  We will have her visit start at Neurology.  Please make sure to bring all the records and films with you.  Good Luck.
