neurotoxicity late effects

I have a complex history:
1983 dx Non-Hodgkins Lymphoma, 19 yo (massive chest tumor, described as the size of a small soccer ball or two enormous grapefruit).  Treated with M-BACOD weekly for seven months. Both Oncovin (vincristine) and Methotrexate make me at risk for neuropathy.  Have always had mild neuropathy, recently symptoms are increasing at rapid rate.  

2005 pacemaker for severe sick sinus syndrome, revealed severe vein fibrosis. Later same year dx with superior vena cava syndrome, aggravated by pacer leads, with 95% occlusion + thrombus.  Multiple venoplasties.  Chronically symptomatic.  Crushing chest pain w/ any exertion which no surgeon or doctor can give a name.  
2006 breast cancer dx.  partial mastectomy.  Stage IIB IDC, w/ extensive DCIS & LCIS.  Unable to take more chemo, had lifetime dosages earlier, and rads contraindicated because of veinous issues.
2007 8 hr venoplasty to open subclavian and place stent, as well as balloon svcs.  
Present: chronic, relentless wrist, hand, elbow, shoulder pain.  Arms fall asleep when prone.  Burned hand on fireplace and never felt it.  Cannot carry plate of food or full cup of tea.  Dropping things.  Dressing and bathing difficult.  Housecleaning impossible.  

Could the crushing chest pain also be neuropathic in nature?  From the massive size of tumor compressing chest stuctures?  Please help.  Need a diagnosis to receive disability...
