orgasm thunderclap headaches

im a 38 yr old woman and recently started having thunderclap headaches right at the onset of orgasm,after the third time this happened i went to the er and i got a ct scan and mri both were normal except the mri the dr said brain changes probly due to migranes (migraines),except i dont get migranes (migraines).im wondering what that means,also i have constant pain on the same side of my head as the thunderclap headaches are on pain behind the eye and ear(i do not have a sinus infection)the whole left side..should i be worried?
