pulsating on left side of head and neck

for 22 yrs. have had pulsating feeling on left side of head and neck that seems to increase in intensity when i work hard or exercise. i have had panic attacks over these years and seems to be when i am tired. i feel slightly of balance at times and want to lay down and close my eyes. when i lay down that is when i really feel my entire left side of my head and neck pulsate at either a mild, moderate, to severe intensity. i have had three mri's in 93, 94, and 98 all with the same conclusions scattered t2 hyperintensity in the deep white matter of the left frontoparietal and temporal lobes. possibility of changes due to demylination. no changes from any of the subsequent mri's. neurologist seems to feel it's nothing and symptoms are pschological . it seems caffeine makes me feel somewhat clearer and focused . klonapin .50 seems to help also . no antidepressants seem to help . please advise me as to what path to take if any . thank you for your indulgence.                                                                                                 bobh7
