recommendation for neurologist in Salt Lake City

I was wondering if anyone has a recommendation for a neurologist in the Salt Lake City area.

Here is a brief description of my symptoms (for the past 6+ months):

- 3-4 migraine headaches (with auras) per week treated with 100mg Topomax (topamax) daily and Frova and Tramodol as needed
- numbness, tingling, and "buzzing" in face, neck, arms, and hands (both sides) on a near constant basis
- inability to place chin to chest (severe pain causing "blackout")
- occasional jerking in exterior eye muscle (left side)

Here's my latest MRI results (as of January 2009)

C2-3: Mild disc degenerative changes with anterior disc bulge,
spurring. No posterior disc herniation or neural impingement.

C3-4: Mild to moderate disc degeneration with a grade I to II
annular bulge, associated mild marginal osteophyte formation and
facet arthropathy. Degenerative process causing acquired left
foraminal stenosis.

C4-5: Mild to moderate disc degeneration with a diffuse annular
bulge posteriorly approximating or abutting but not effacing the
ventral cord. No frank foraminal stenosis. Mild uncovertebral
spurring and facet arthropathy.

C5-6: Mild to moderate disc degeneration with a diffuse grade
I-II annular bulge. Mild uncovertebral spurring and facet
arthropathy. The posterior disc approximates but does not
overtly impinge on the ventral cord. No foraminal stenosis.

C6-7: Mild to moderate disc degeneration with a diffuse annular
bulge posteriorly approximating but not overtly impinging the
ventral cord. No foraminal stenosis identified.

C7-T1: Normal.

IMPRESSION:
1. Multilevel mild to moderate disc degeneration with broad
based annular bulges most pronounced C3-4, C4-5, C5-6, and C6-7.
The posterior discs approximate and at some levels abut but do
not efface the ventral cord. No frank central stenosis.
2. At C3-4 there is acquired mild left foraminal stenosis.

I have tried acupressure, deep tissue massage, meds (none of the other migraine meds work, I've tried them all), heat, ice, TENS, physical therapy . . . I'm going nuts!  It's 6 weeks until I can get into my GP just to discuss my MRI then who knows how long to find a good neurologist.  I can't wait until May or June to get to a neuro.  Anyone have a recommendation in the Salt Lake area or suggestions of what else I can do to help alleviate some of the pain??
