right shoulder grinding and aching

hi i am 20 yrs old and my right shoulder has been aching for 4 months now it makes a gringing noise when i move it, when i was 15 and done long distance running i had very bad pain in my right shoulder it was like a stabbing pain, i have been to physiotherapy and have been on anti-inflamotrys for 2 months and it is still no better, physio cant do no more as they cant find a problem and doctors are not sure what it is, there is arthritus through our family and my sister had it when she was 7 and has hap hip and knee replacements, cen anyone help me to find out what it might be. thanks shelley
