seizures

My wife has seizures normally in the evening when watching T.V.. But she also has had them while cleaning the pool on a sunny day and driving past caution lights. I'm pretty sure it's caused by light flickering. The neurologist put my wife on Topamax, but it's not very affective. What medications do yuo suggest and is it possilbe that the seizures will just go away? My wife is 37 and did not have a seizure until age 35. Why is this starting now? My neurologist is useless and I'm running out of options. Cat scan,MRI, EKG all show nothing. Should she go to a specialist and what good would it do. What will they do that is so different from any other doctor. Like I said, I already know what triggers the seizure. Can they map it out and possibly perform surgery and remove the problem? If so what are the risks involved?My wife does have headaches,fatigue,enimea,and occassional UTI's. Any suggestions would be extremely appreciated. Thank you for your time
