severe headaches/roaring in right ear

Hi am 31 and about 6 months ago i started having problems with my right ear. I bent down quicly to pick up something off ground and my ear clogged up like i had fluid in it. I wasn't too concerned cause this had happenned to me before and always eventually cleared out. Well, it never has. I have been to ENT and there is no fluid in my ear, he said seems like i've had an inner ear shift? I just quit going cause he wasn't helping me and i was waisting my copays.My sound in right ear is very muffled and i always hear this womping sound. Its just getting worse, and now i have these constant headaches everyday! They are really interfearing my life. I am extreamly concerend cause i have to take tons of Tylenol to get through every day of my life, and that can't be good for me. I cant  stand high pitch noises, it makes me cringe. I would feel better if i had a CT scan or MRI just to rule out anything major, do you think thats what i should do? Someone please giove me an idea of where to go and who to see for this problem.
