seziure

seziure

My daughter of 1 year of age has the history of neonatal seziures.
when she was 7th day baby, she had an jerk and doctor prescribed the Gardinal 30Mg (phenobarbitone tablets i.p) for three months, she was fine till 7 months.
Later in the seventh month she had a high fever as well got the seziures back (she used to bend her neck front as well her both arms) . Then doctor examined with EEG and blood report, came to conclusion as MYOCLONIC seziures and prescribed VALPARIN 200(sodium valproate oral solution i.p) , B Long (yridoxine sustained release tablets) and B-complex with vitamin c syrup called Becosules.
she was fine and normal had two or three jerks, then as a routine check up in the 11th month, doctor prescribed to continue with the VALPARIN and NORMA BRAIN (2-pyrrolidone Acetamide Syrup) as new one instead of Blong and becosules.
Until 11 and half month is was fine then suddenly she started to get the seziures continously for 10 to 20 times a day..(she use to lift her left shoulder and use to bend her neck front for 2 to 3 sec).
Now she is of 12 months old, doctor has told not to use NORMABRAIN  and to continue with the Valprin,Blong and becosules as the earlier one.
i would like to know what has been the cause is this because of change in the syrup of Norma brain.
As well is there any permanent cure for this. can u please suggest..

Thanking you,

Hi.
Seizures can be controlled using various anticonvulsants like phenobarbital and valproic acid.  The use of these drugs depend on the type of seizure.
Uncontrolled seizures should be investigated.  Doses of the drugs should be checked because this is dependent on the weight of the patient.  Also, there are certain drugs that interact with the anticonvulsant, thus making the blood concentration low, resulting in seizures.
If your daughter still continues to have uncontrolled seizures with her present medications, it is always advised to have her reexamined by her physician.
Good luck.

Hi,
I think if seizures are recurring, they need further investigation as to see the level of anti-epileptic in body.
If the level of Sodium Valproate is low (anti-epileptic) then you have to increase the dose. If it is normal range then you have to add on to other anti-epileptic.
I would also want to know, was there any other cause of seizures at 7 days of age like any biochemical abnormality? Whether any EEG was taken then? How long was phenobarbitone continued?

Hi,
Nice to know that physician has revised the dose of medications.
I want you to send the trough levels of valparin, like if she is due to take her medicine at around morning 9am then send blood investigations at 8am and give her dose after test is done.  Also if possible, if you can do her biochemical studies, then it will be helpful for further reference.
Well seizure can be controlled if medications are given in appropriate, timely doses.
Make sure she is growing well, weight wise, height wise, nutrition wise etc.

thank you,
Seziures are still recurring from past 5 days but it has reduced slightly. physican has adivced for blood check to know the level of dosage. Seziures occurs more as soon as she wakes up from the sleep.

There was no biological abronmaility or any such thing. The first seizure occured on the 7th day when one of my friend pulled my daughter nose, suddenly she couldn;t able to breath n just move here mouth n eyes towards right side for 3 to 4 sec.
EEG was not taken on the 7th day .physican just gave the medicine n continued for 2 months n later gradually decreased dosage for one more month n stopped.Till 7th month thr was no problem at all.

Hi,
Can you please tell me at 7th month what investigation's were done to find out the cause of seizures?

hi, sorry for the late reply,
At the 7 months of EEG has taken and the report says that the background activity in PC and FT leads consists of mixed frequency of theta 6Hz (50-70 uv), low voltage activity, well defined sleep spindles and delta 3 hz (90-100uv), bilateral symmetric. At number of times, there are bilateral post central slow wave runs at 180-200uv.
conclusion: EEG is mildly abnormal, findings suggests bilateral post central dysfunction.
As well there was METABOLIC WORK UP, DIFFERENTIAL COUNT,URIN AMINO ACID SCREEN tests has been done and the results were negative.

Now she is almost fine. hardly once in a day it occurs, she is recovering i feel . And the Blood test of Valproic acid contents has been done. The results says 79.5mcg/ml . it should have the range from 50-100mcg/ml... PHysican has prescrbed total 10ml every day dosage.

Hi Ravikanth,
I hope the best for your daughter.
I asked you about investigation because I knew there are several other conditions in infants to be ruled out before starting an anti-epileptic.
The results are proved negative hence there are no other metabolic causes to your daughter’s seizures.
Next time when she has seizure, the line of management after discussing with your daughters neuro-physician will remain same.
Bye.
