tingling-burning arms legs face

I am 44, female and was is very good health until 10 months ago. I have tingling and burning in both legs beginning just above the knee and going down through my toes, both wrists,left forearm and around the eye orbits. I had a cervical fusion at C4-C5-C6 in 2000. I have been working with my GP for 5 months to find a cause. She has run blood work for vitamin and mineral deficiences, thyroid, autoimmune diseases, sedimentation, diabetes and had an MRI of my brain, cervical spine, and left shoulder. Blood sugar (fasting) was 85 (my father had diabetes and neuropathy). Sedimentation is 40 while it was 8 a year ago. Autoimmune test came back with ANA of 1:164, but further testing found levels at the high end of the normal range. I saw a neurologist who found a 50% loss of sensation in both feet during an exam 5 months ago. I occasionally have a tremor in my left arm or hand. I do have a report which sees a tear in left rotar cuff and deltoid. I will be seeing an orthopeadist this week. Physical therapy only made the symptoms worse and the therapist stopped it. Along with all this my weight has been creeping up. I lost 75 pounds nearly 4 years ago and kept it off for 3 years,but suddenly the regimen I was following (small amounts of nonprocessed carbs and lean protein 5 times per day) stopped working. I take vicodan and amitriptyline to make it through the day, but they really don't help and I would rather not take them. Last month I landed in the hospital with a suspected heart attack. They told me I had an abnormal EKG (flattened T-wave). I have no cardiac risks so that is very surprising. My GP ran a urine panel after this to see if my organs were breaking down, but there was no protein, blood or infection in the urine. Help
