what should I expect now that I have a recurrent small carotid dissection?

After a minor car accident in 2002, I developed a small intimal flap in my right ICA.  After 9 months of coumadin all seemed to be well.  Now after another minor blunt injury to my R neck, it seems that the dissection has recurred/perhaps never fully healed.  Luckily I've had no neuro sequelae with either episode.  I'm staring lovenox/coumadin again, but don't have a good sense of my stroke risk, what I should and should not be doing in terms of exertion, etc.  I asked my vascular surgeon, but she was quite vague.  Also, what do you know about stenting?  I am a physician myself (dermatologist), and find the lack of info about what to do or expect very frustrating.  I've done some literature searches, but perhaps I'm just not getting to the right info?  I'm otherwise healthy, with 2 small kids, and scared.

I was disgnosed with a left carotid dissection six months ago. Following the recommednation not to over-exercise I maintained an irregular program of slow 30 min jogs keeping my heart rate to less than 130 bpm. This level was not specifically suggested by my Specialist - more of a pragmatic limit for a relatively fit 46 year old. Last week I raised the level to be less than 30 mins at less than 140 bpm. This lead directly to TIA with symptons of numbness to the right side of my face/tongue and weakness of the right arm. These symptons were much reduced after 4-5 hours.

I guess that inadvertently I established my exertion limit
