white matter lesions

Hi, I am a 45yo/ w/female. Started having bad headaches which are suspected migraines about 18 months ago, usually peri-menstrual. Have progressed to an almost constant underlying headache for the last 4 months. I also had a supsected seizure in Sept last year. Had a CT scan at the time of seizure and there was nothing suspicious on that. Tested for epilepsy - all ok. Have been increasingly dizzy lately and feel like i have to concentrate much harder than before to think about anything (like typing this)...
Had MRI last week - results (quote report):"considerable number of white matter hyperintesities in both cerebral hemispheres. No abnormality in brian stem or cerebellum. WML are subcortical and periventricular, not on corpus callosum. Largest WML is in periventricular region of left trigone measuring 15mm. Remainder of brain otherwise normal. Partial empty sella in pituitary region. MRV demonstrates normal appearance. A focal cortical lesion is not identified but considerable whtie matter abnormality appears to be present. ?small vessel disease, migraine?would be atypical for demyelination." UNquote.
Can anyone interpret? Is MS a possibility? Is it just migraine damage? I don't understand the "atypical demyelination" bit....and if it's "small vessel disease", why would the MRV be normal?

Hello Dear,

Because of the inflammation and breakdown of the blood-brain barrier in multiple sclerosis, lesions, the presence of extravascular fluid leads to hyperintensity on T2-weighted images. Thus, in a patient with MS, MRIs typically demonstrate more than 1 hyperintense white matter lesion.You should consult your neurologist and get a confirmed diagnosis.
