year long headache in back of head

My 6-year-old daughter has had a headache in the upper back of her head since May 2006.  She has been under the care of a neurologist since then.  The headaches seem to be more painful and more frequent recently.  She just woke me up to tell me her head hurt really badly but now she is sleeping beside me.  We are starting our second round of tests to find a cause.  She was diagnosed with migraines but the headaches have responded minimally to all migraine medicines approved for children.  She took 5 different ones but is currently off of all medicines except for occasional Motrin.  Her blood has been tested thoroughly twice and was again normal.  Last year, the rheumatologist and gastroenterologist ruled out causes from their areas.  She is to have an MRI next week again.  We are going to the opthamologist to examine her optic nerve again and an allergist for the first time in 2 weeks.  If the MRI is clear, she will have a spinal tap to determine if the spinal fluid pressure is too high.  Any ideas on how to help her?  I am very worried.
