17 month old losing weight, muscle tone and has sagging skin

My 17 month old daughter has been loosing weight since she hit 12 months.  The first year she stayed in the 95th percentile for weight; Now she is below the 20th.  The thing that worries me more than her weight loss is that she seems to be losing muscle tone.  Her skin is loose and saggy.  She has the apperance of an "old man"; I am sorry but that is the only way I can think to describe it.  Her body (bottom, upper arms, thighs, knees) just seem very droopy.  She eats well and we have met with a nutritionist who does not believe the problem is with her calorie intake.  What are some conditions that I should ask her doctor to check into?

Suggest having an MD test her to see if there are any medical causes which can be treated. If there are no medical causes, then suggest giving her Pediasure or milkshakes (milk and ice cream blended) in place of milk for extra calories / protein to help her gain weight. Here are some additional tips for extra calories/protein: If she likes pasta, then add meat or cream sauces to the pasta instead of plain tomato sauce or plain pasta. Add dry milk to oatmeal, mashed potatoes, gravies, creamed vegetables, and creamed soups (add additional meats to soups).
