CP with low apptite

Hi,
I have a 18 months old daughter. she is hydrocephalus and has sever  CP. she has low apptite. Please help me to increase her apptite.
she gained weight only 0.4kg during 5 months.
Her one day menue is here:(all is puree)
In the morning she has 3 or 4 once milk(smilac advance) and after one hour she has around 100ml cereal with whole milk(one day with youlk egg and one day with half walnut and 3 almound)
and around noon she has 100ml mixed meat, vegitable,rice or nodle. and 2 once milk
afternoon 120ml fruit (mixed benana, apple, kivi, pear and orang)
and she takes 50 or 60ml mixed meat, vegitable,rice or nodle.
and in the night befor she going to bed she drinks 2 once milk( sometimes nothing).

I am worried and concerned if the above food is ok for her and does she get all the required nutritions from it.since her quantity of intake is reducing day by day.. I am more concerned about that her poor appetite might cause poor health or a nutritional deficiency.

Please advise me for her best.

Her weight is just below the 10th percentile and height just above the 50th percentile for her age. Her weight is not off the chart which is good. For an 18 month old you need to switch to Pediasure as the milk source and not baby formula. An older child needs more then baby formula. Use Pediasure in place of milk and the formula which will give her extra calories and protein.
As a guideline for an 18 month old child should eat the following:
Dairy 4 feedings a day – Milk-use Pediasure (4oz), cheese (½ oz), yogurt (½ cup), or cottage cheese (¼ cup).
Starch 4 feedings a day – Cereal (¼ cup), pasta (¼ cup), rice (¼ cup), bread (½ slice), or crackers (2).
Fruits & Vegetables 4 feedings a day – Fruits (including 100% juice limited to only 4-6ozs) & Vegetables (¼ cup).
Protein source 2 feedings a day – Meat or fish (1oz), egg (1), or beans (¼ cup).
She should eat 3 meals and have snacks between meals using the foods above to be spread out throughout the day. Hoped this helped you.
