Nutritional Deficiencies

I just did nutrition testing for the following:

Vitamin B1
Vitamin B2
Vitamin B3
Vitamin B6
Vitamin B12
Folate
Pantothenate
Biotin
Serine
Glutamine
Asparagine
Choline
Inositol
Carnitine
Oleic Acid
Vitamin D
Vitamin A
Calcium
Zinc Copper Magnesium
Glucose-Insulin INteraction
Fructose Sensitivity
Chromium
Glutathione
Cysteine
Co-Enzyme Q-10
Selenium
Vitamin E
Alpha Lipoic Acid
Vitamin C

I was determined to be deficient in the following (and started these treatments):
Vitamin B3 (25mg in Ortho Molecular Alpha Base vitamin)
Vitamin B12 (500mcg in Ortho Molecular Alpha Base vitamin, plus weekly injections)
Vitamin D (5400 IU in Ortho Molecular Alpha Base vitamin and additional Ortho Molecular supplement)
Zinc (10mg in Ortho Molecular Alpha Base vitamin)
Iron (Tandem Plus)

So three questions:
1. Are there any other nutrition tests that you would recommend based off of these results?
2. What could be the underlying cause of all of these deficiencies?
3. I've also been tested positive for low thyroid hormone (appears to be Hashimoto's from a thyroid ultrasound, but I'm still waiting on antibody blood tests). Could nutritional deficiencies (weakened immune system) cause the thyroid condition and therefore cure it with proper supplements?

Suggest an Iron profile blood work to determine which type of deficiency you have due to the decreased levels of Vitamin B12 and Iron so that you are treat correctly. For example Vitamin B12 decrease with Folate deficiency and the treatment is Folate supplementation not Vitamin B12 shots. Eating healthy 3-5 times a day would help to keep the body healthy and not have vitamin / mineral deficiencies unless there is an underlying illness which may interfere with the body’s functions and deficiencies may arise. The recommended treatment for low thyroid function is thyroid medication and routine checking of the thyroid levels.
