gain weight

i am patient of ibs and i am not gaining my weight .from the last ten years my weight is constant,when i wake up early in the moring my mouth thats full of salvia and my urine make bad odur .i also get a pain in the middle of my chest when i sleep on side then i get a pain but when i stretch my arms i get a tak tak sound in the middle of my chest n with that i feel better.Now i am not sure why?i get this pain.In past i was smoking , in high school i punched by friend on my chest. plz tell me to whom may i consult for this.
