Hey here are the symptoms, what do you think it could be?
55 year old hispanic male,
presents with left shoulder pain, pain present for about 6 months, becoming progressively worse.
Points at the top of shoulder
Pain is dull ache, worse with motion. Better when resting at his side.
Working over his head makes the pain the worst.
No specific trauma recalled that relates to the pain.
Pain radiates the side of arm to about the elbow. (lateral aspect).
Occassional wake up at night with pain in the shoulder.
No numbness or tingling.
Past med hx, tonsillectomy at 5, appendectomy at 25, no other sx.\
Only hospitalations were for tonsil/appen
Takes propanorol for hypertension, taking for 15 years.
Occasional motrin, apring tynol for shoulder pain, take edge off pain but don’t make go away.
1-2/10 at best, 6-7/10 at worst.
Current on all immunizations and vaccinations.
Denies any hx of malignancy, no fam hx
oth parents have heart dz. Strong hx of hypertension.
Not diabetic hx in family
Denies hx of stroke, kidney dz,
Parents smoke, he smokes, 1 pack to pack ½ a day for 35 years.
Does not drink alcohol, nor elicit drugs of any type.
No known allergies.
No trouble breathing.
Construction worker, can still work
Right handed
Tpr= 98.2
76, regular rate and rhythm, 20 in nonlabored.
136/84 bp when seated.
Skin is pink, warm and dry.
you didnt mention any physical examination findings, but my two best guesses are AC joint arthritis, or rotator cuff promblems (either impingement or a tear)
