comminuted intra articular r distal radius fracture question

After an auto accident in September diagnosed as a severly comminuted intra articular fx of right distal radius. I had ORIF surgery 5 days later. After many months of PT still pain especially ulnar sided. Diagnosed with ulnar impaction syndrome as the Radius shortened. There is a question of a screw possibly going into the joint, as I have pain when curling my wrist inward. Just had a ct and on the 3d view there is a piece on the dorsal side that appears to be malalligned. Is this a real problem or not? see picture

This sounds like a very real problem. At this point, it sound like further surgery is needed, and sooner, not later while more damage is done. At the very least, removal of the screw, combined with an Ulnar shortening and possible radial osteotomy. If your surgeon is not well experienced in difficult hand procedures, it may be wise to consult an orthopedic hand specialist.
