Certainly a bad contusion about the shoulder can be a sinicant cause of pain. However, not all injuries to the shoulder are visible on x-rays.  For example, a torn rotator cuff, or a torn labrum can both cause significant pain. Along with his inability to raise his arm, I would be concerned about a torn rotator cuff. If his symptoms persist, an MRI would be helpful.
