Dislocated and Fractured Elbow

My daughter dislocated and fractured her elbow last week (bone 1 inch out of socket for aprox 6 hours because she ate right before it happened - age 16).  Hospital re-set it and put her in a soft cast with sling and pain killers.  We were told arm might be swollen 5 - 7 days and some bruising.  It's been 7 days, arm still very swollen and bruised, fingers swollen and she's in a lot of pain with or without pain killers (oxycodone/APAP 5mg-325mg).  Orthopedic doctor office said don't come in for appointment until swelling is gone.

If arm is still very painful and swollen/bruised after 7 days, is there another type doctor she should see or should we wait it out for orthopedic doctor until swelling is down or should we go while it's still swollen?  Also, does she need her  pediatrician or an orthopedic pediatrician at this time?
