i have herniated disc L-4,L5 L-3 dimenssion 3 mm , I am in my second epidural shot, and I didn't see improvements, actually I feel worse, numbess. pain, tingling, legs and foot, and left arm, and back pain, do I am going to need surgery  after my third shot if my symptoms persist

Caregiver is correct -- we don't know how you're going to respond to care, however, three shots, and you're out -- don't go for any more. Three injections should prove or disprove efficacy of injections.

Surgery should always be reserved until all other conservative means of treatment have failed.

Maybe yes, maybe no. The epidural shots never cure anything. You need to go to a physical rehab program before anything else. You have to go in person. You can't learn the exercises on youtube.
