Lower Pain

i had a lower back pain, but its focused on the left side of my buttock and goes down to my leg. pain is not strong you can say its 3 out of 10. i sit in office a lot but at night i usually exercise, i do biking, Cardio gym (push ups, treadmill, squats, stomach), soccer, tennis and sometimes swimming. i live in Nigeria, so when i felt the mild pain i went to a family doctor becuase  i was afraid that sport will make it worse he told me to take Voltaren 100 MG  one tablet morning and evening and another medicine spiraled (muscle relaxant) one tablet three times daily after eating for five days. after five days i stopped the medication and rested  for 10 days, still the pain so he told me to repeat the medication so i did and rested for like twenty days and somehow i didn't feel the pain again only sometimes every couple of days. so i went to exercise again starting with swimming and biking then moved later stage to gym and soccer and i was alright for two weeks then felt the MILD pain again. since no specialized doctor i decided to go myself and do an MRI and had a report i will attach for you. please help what medication is required or there is for physiotherapy but pain is not strong so i am not sure i need it, and can i continue with sport as sitting down i feel the pain but while excesses i feel perfect.
