Lower back pain

I had first had a severe lower back muscular pain in Oct 2007 while doing sitpus with weights in the gym which subsided with regular ointment & pain relief spray to the area although it took more than a month. 2 months later in Dec 2007 i hurt again in the lower back region while spot jumping & had a sudden deep pain in the lower back. This pain spread in the whole lower back region. Slowly the area of pain has shrinked to a smaller area now but a deep burning pain to the lower left side of the lower back close to where the pants belt would be, it seems very deep inside & never on the muscles. I dont feel the pain when seated or sleeping, but just 2 minutes of standing  creates a burning pain to a spot deep inside at the lower left side, this pain seems to stretch till the left thigh & knee portion. On the surface i am unable to pin point the exact area of pain. If i had been standing for long then this pain persists even when seated or sleeping.

In addition around 9 months back could be around june-july 2007 i started feeling pain in the left knee, somewhere inside couldnt locate it anywhere on the surface around the knee portion, a pain that feels like a typically feels like a heartbeat continuously beating off & on with the intensity of the pain whenever it happens. This was only occasional. I never had a leg injury or knee injury in the past. Also i should mention that i had had history of lower back aches as part of sports injury mostly as i had been on & off to gym specially during the situps with weights. The 1st doctor i refered to was of the opinion that the back pain was because of muscular spasm & the knee pain due to a minor menscal tear.The MRI scan as adviced by him said the following..
1. myoxide degeneration in posterior horn of medial meniscus
2. mild intra-articular effusion

A 2nd doctor said both were due to muscular spasm of the lower back & has suggested physiotherapy with medicines. I wish to understand if this isnt serious or if serious then how serious.Would normal back excercises in the physiotherapy suffice ? Also no amount of hot water bag treatment to the lowerback or topical ointment to it has helped in anyway. Presently i am taking medicines(no ointments, or hot bag treatment) alongside physical exercises & SWD (Short wave diathermy). I had also done IFT sessions, still i dont find any visible improvement in the intensity of pain. Has it to do with nerves being pressed badly. Need your reading on this.
