Lumbarized S1 with bilobed transverse Process

I am trying to find cases where folks with lumbarized S1 segment with large bilobed transverse process which creates a pseudoarticulated joint between the sacrum and the ilium.

I have had 3 fusions which resulted in very solid fusions between the sacrum and L6-S1 to L4, L3. Later a left SI fusion as attempted with a metal plate, but this fusion failed. After the failed SI fusion, i went on long term medications to deal with the spinal pain.

During the SI fusion, the left bilobed L5-S1 transverse process distal portion was found to be solidly fused to the sacrum, but the proximal lobe was in free surrounding scar tissue. Considerable motion was noted between the proximal lobe and the ilium, as well as significant motion at the SI joint.

My question is that since the SI fusion failed, wouldn't it be feasible to cut the proximal bone/lobe of the L5-S1 transverse process off in order to not create a pseudoarticulation joint between the ilium and the transverse process. And what Drs would be best to contact regarding this.

My chief complaint is hard aching pain at the location of the false joint, and aching pain down my left leg only to the ankle. The pain meds have simply made my life worse, and tolerance has built up over the 17 years i have been on the opiates. I have had excellent medical help in the past, just would like to know if cutting the deformed transverse process bone off would be something a Dr would consider. I no longer live near the Doctor who performed the SI operation.
