My elbow is constantly tense, locks, and is is pain - what should I do?

I played football and baseball growing up as a quarterback and pitcher/infielder. I constantly felt elbow pains, but I just ignored them and thought of them as regular soreness. All I did was ice them occasionally. The real pain started in my first year of high school playing football. Just warming up with my fellow QB's caused intense pain to the point where my elbow was numb halfway through every practice (4 times a week). I did the same as I always did. After the end of the season, I couldn't deal with the pain and decreased throwing distance anymore - so I quit. Four years later, I always have a tenseness in my elbow, and it locks regularly too. Occasionally, sharp pains with quickly shoot through my elbow and it also slips every so often. Even throwing a football at the beach a few times with hit me with a dull pain. I went to the doctor after I quit football, but they said everything looked fine and that it was probably just me using by arm instead of my whole body to throw. They also said that with a break of a few months of sports that my elbow would be 100%, but 4 years later here I am. Any advice?
