Options after hip fracture

Options after hip fracture

I am looking for a medical opinion on the following situation:

- 83 year old woman, physically strong, but with COPD, osteoporosis, former heavy smoker (quit 10 months ago)
- hard fall three and a half weeks ago resulting in badly fractured right hip (not a clean break, but rather a middle section with multiple fragments of bone in between the two [hopefully] more solid pieces).
- surgery to repair damage 3 weeks ago (steel plate, 6 pins and one screw "into the neck of the hip", which is, as I understand it, actually part of the femur bone)
- post surgery fall and potential damage which is causing "shifting" of the screw (up to 3/4" movement, according to the surgeon)
- latest x-ray results convinced surgeon to extend the "no weight-bearing" timeframe to 6-8 weeks from surgery (so another 3-5 weeks of wheelchair mobility).  He was very clear that ANY weight-bearing could negatively impact the bone healing.  He was also clear that if the healing did not occur, a total hip replacement was not an option due to the location of the break ("too low").
- my understanding is that due to the osteoporosis, and the possible damage done after the surgery, the screw might work its way out of the bone, and "pop" free.  If this occurs, and if a hip replacement can not be done, what are the other options?  Are there any?

Thanks
