As he has a rebreak and this can be due to either reason. Keep the cast for 6-8 weeks and he should have a follow up X-ray after 6 weeks and then the final decision to keep the cast till 8 weeks or not.
Give him proper balanced diet with protein and calcium and go for the rehabilitation very slowly and under an expert.
