Tkr with popping? Normal?

I had aTKR to my left knee on May 5th. Suffered a blood clot 3 weeks out. I am 42 years old, endured 17 prior knee surgeries over the last 20 years. I have an Oxynium knee because I am allergic to nickel. I started therapy not even 24 hours after surgery. Stayed in the hospital for 7 days. I went home with a CPM and was using it daily for 8 hours a day. I also attend physical therapy 4 days a week. I am on a 50 and 12 mcg Duragesic patch that is changed every 72 hours. I also take 350 mg of Soma 3x a day, Lyrica 75 mg 3x a day, Robaxin 750 mg 4x a day, Norco 10 mg 6x a day as well as asprin  and stool softeners.  The pain is unbearable still. Prior to my surgery I was on a 25 and 12 mcg patch, Soma and Norco 4x a day. The knee is stiff and will only bend to 78 degrees on the CPM. My extension is -5. I am now experiencing a popping of what seems to be the patella which was also replaced on the underside of it. It is painful but not as much as the excruciating pain I have on the medial side of the patella. It is also hot and red in that area and causes it to where I can not bear any weight at all. Although last week I was able to walk with a cane. It is now 5 am and I can't sleep due to this pain. My doctor is 200 miles away from me and I don't want to sound stupid to him about this problem. Though I fear something is wrong. Any advice would be appreciated. Thank you for your time.

PS
I have suffered infections numerous times after surgeries and always seem to have things go wrong that shouldn't. Also this wouldn't be a sign that my body is rejecting the TKR would it?
