Tuberculosis in TB

Sir,
     Since last 18 months I have been suffering from pain in my testiclesTesticle lump
Testicle ultrasound
Testicle pain, after diagnosis only bacterial infection was found but there was no trace of TB since all tests failed(semen-PCR & other tests) I was on anti-biotic for almost one year but the pain continues without any trace of TB. then this January my chinChin augmentation
Chin augmentation - series grows up(nodular expansion) after diagnosis TB was confirmed and I was on AKT4 for 3 months and chinChin augmentation

Testicle ultrasound
Testicle pain(epididymis). Then I   on R-Cinex 600. During this whole episode initially my semen count was normalNormal saline flush, then it comes to nill now. Plus the semen culture showed heavy growth of bacteria(which is normalNormal saline flush now). Kindly advice me what to do now, since I am looking for second opinion.
