about my father

hello doctor i wanted to take an advice from you, my father 68 yr old male with history of diabetes type 2 sice 30 yrs along with  chronic kidney disease.(creatinine 1.6). he had fracture of his right hip ball 4 yrs back underwent surgery  immediately hip ball was not replaced though.later he was alright and could walk without much difficulty but from the past 1 yr he is having pain and difficulty walking. he has already become bedridden now.we consulted an orthopedic surgeon who advised for partial hip replacement but his physician looking at his age and complications advised not to take him for surgery. he is really depressed because he cant walk. is there any other way that might help him doctor other than surgery. plz doc help.he is into severe depression

Well, with the description this looks further degeneration/ fracture or avascular necrosis. I am not particularly in favour of ‘partial’ joint replacements and these are getting replaced with total replacements for better prognosis and lesser complications. Anyhow the benefits of doing a surgery would need to be well evaluated against the risks and while the conditions may not be absolute contraindications, these would need to be controlled for a better outcome. I would suggest consulting a nephrologist and an endocrinologist for appropriate management of the primary condition before seeking an opinion with an orthopedician and an anesthesiologist for suggestion of an appropriate management plan.
