chest and shoulder pain

i've been experiencing on and off pains in the chest & upper left shoulder for 5 months.
i'm 21 & have been working out for the last year and a half.
Thought it was osteocondritis at first & went to see a orthopedic; he couldn't find anything wrong with me and for precaution asked me to get an E.C.G done, which i did...everything fine( !! ).
I have been a smoker for the last 4 years ( around 5 a day ).
whenever i have a smoke the entrie shoulder area and the center chest region feels sore & develops cramps for around 3 to 4 hours ...then eases out. I also experince a tingling sensation in my left hand.
The area below the left shouder always feels sore & sometimes i get a cracking sound from the chest whenever I breathe heavy.
I have a major sinus problem and am always sneezing and coughing year round. Just around the time this problem started i had experienced shortness of breath for 2 days, but thought it was due to the heavy cough and cold.
Not had a single cigarette for the last 2 weeks but still am nervous.
Is this an external injury of the shouder or is something seriously wrong with me?

sounds lung related partially due to smoking most likely, possibly asthma? you can get chest pain from asthma, good thing you have stopped smoking. when you have the pain in your chest try taking asthma medicine and see if it helps. You may have also developed a costochondritis from the coughing and added pressure, this is not life threatening. I'm not sure if that can go into the shoulder, if your shoulder cracks to it might be stiff from tension, see a physiotherapist, he might recomend massage therapy. If it is asthma it has to be controlled so see your doctor and tell him or her exactly what you told me.

Hi,
I am afraid that your complaint does not relate to any external injury to shoulder  and chest wall. It seems to be related more towards the chest/ lung pathology.As  your smoker for a longer duration and shortness of breath of recent onset with added sounds do suggest lung related pathology probably pleurisy.
Hence you need  to consult a physician to rule out these problems.
