sounds lung related partially due to smoking most likely, possibly asthma? you can get chest pain from asthma, good thing you have stopped smoking. when you have the pain in your chest try taking asthma medicine and see if it helps. You may have also developed a costochondritis from the coughing and added pressure, this is not life threatening. I'm not sure if that can go into the shoulder, if your shoulder cracks to it might be stiff from tension, see a physiotherapist, he might recomend massage therapy. If it is asthma it has to be controlled so see your doctor and tell him or her exactly what you told me.
Hi,
I am afraid that your complaint does not relate to any external injury to shoulder and chest wall. It seems to be related more towards the chest/ lung pathology. As your smoker for a longer duration and shortness of breath of recent onset with added sounds do suggest lung related pathology probably pleurisy.
Hence you need to consult a physician to rule out these problems.
Good luck
