chest muscle strain, sprain, or rib fracture?

I was in the back of my closet hanging clothes when the bracket pulled away and shelving, storage boxes, and a ton of heavy clothes (clothes bar and all) came tumbling down in two phases.  It happened so fast- I may have tried to turn, lift my right arm to instinctively protect my head.  I was more interested in the bruise to my forehead at the time, but the next morning I had pain across my chest to underneath my right arm (rib area), mostly when I breathe (ha). I thought it was muscular and actually proceeded to work in my garage that next day.  By nighttime, it was much more pronounced.   Tomorrow will be 5 days and it has not improved.  It hurts severely to cough, sneeze, and breathe. I have not been to the doctor because I am without insurance right now.  I've taken some advil and prescription strength ibuprophen I had and it relieves it only barely. I can lift my right arm over my head with no trouble, but I have more pain when I apply pressure downward with my right arm, such as cutting an orange or pulling up on something in a lifting motion. Since the pain is mostly in my chest area, could it still have to do with a rib fracture ?  How long should I give a healing time without concern that I need to get to a doctor immediately?
