hello doctor. I am Bikash From NEpal. I have been passed through the surgery of back in the spine called L2 and L3. At there my bones were in destructive condition and there was psoas abscess too so doctor Basanta Pant one of the head surgeon suggested to have surgery so i did my surgery before 2 months and taking TB medicine also. I want to know that when my back pain totally goes out and i will be normal  i m getting my back pain and my legs are not getting full strength while walking so doctor i want to knw that i will be cured or not i m only 23 so plz do help me.
I recently got my MRI and it was found that the approx 4.0*3.5*2.5cm size collection seen in right psoas abscess. At before test of MRI at operation time it was found aaprox 11.0*4.5*5.0 cm size of psoas abscess was noted. Now, after examining the latest report Dr pant said that there is not possible to take out the psoas abscess and he too concluded that this abscess will be eliminated by the tb drugs. and TB consultant has provided me two types of tb drugs 1. Ethambutol 1200 mg and 2. Isoniazid 300 mg and rifampicin 300mg . He said to have these drugs for 1 and half years. I think these drugs can make me well, isn't it, sir?  i m getting my back pain and my legs are not getting full strength while walking so doctor i want to knw that i will be cured or not i m only 23 so plz do help me. Will that poaas abscess will be eliminated by the these drugs? Plz do help me sir i am really worried due to the formation of psos abscess again. Sincerely.

Hi Bikash!

For spine TB WHO recommends a regimen of 2HREZ/4HR3, which involves isoniazid, rifampicin, ethambutol, pyrazinamide daily for two months, followed by four months of isoniazid and rifampicin given three times a week. In countries like Nepal and where an abscess has developed, it is important to continue the regimen sometimes for 18 months. Also, ethambutol may be continues for long. Please ask regarding inclusion of pyrazinamide into the regimen if you have not taken it already for two months. Sometimes amikacin and streptomycin needs to be added to avoid drug resistance. Ask regarding this also.
Along with this, you need to take a high protein diet, eat healthy, drink plenty of water, fresh fruit juice and soups. You need to do exercises to strengthen your back (please do this after consulting your doctor and under a trained physiotherapist). Take care!

hello doctor,
i recently has major problems on my legs. I am not able to walk properely and move myyy legs fingers completly specialy of right leg. I am reaally worried it has become worst and worst day by day. What can be the done for this sir?
Before 2,3 weeks i Falled down accidently on the ground and from that moment this problem has become the major for me although there was no any defect in the nerves and bones through the latest mri results. But I think I am in serious trouble.plz do help me
Before falling on the ground my legs werent powerless as much as this is now. After that accident i layed down on the bed for more than one weeks and after that i recently found my weakness in the legs. Is it due to the accident?

Your back pain will go once TB is completely treated. Sometimes residual pain remains due to deformity. Yes, TB can be cured, but spine TB is a bit difficult to cure and needs prolonged treatment. Complete cure may take up to two years. However, only the doctor who is following you up can comment for sure. You can take a second opinion at a TB center or a spine center of repute in your country.

Thanks doctor for your quick respone.
at the beginning of my treatment I was given only Ethambutol 1200 mg because my Liver was infected. both SGOT and SGPT was increased. later on i was recently provided with isoniazid 300 mg and rifampicin 450 mg with vitamin B6 when my liver functioned well.
I am really worried because my back hurts so much sometimes. So plz doctor provide me with information that:
When my back pain goes out?
Will it be cured?
For how long it takes to be fully cured?
