unilateral ankle edema
hi,
i have swellimg of the right ankle without pain. Im 23 years old a medical intern and i notice the swelling after long periods of standing. Ive seen a dr and lab test like lupus profile, Rheumatoid factor, serum uric acid are normal. xray of the joint is also unremarkable? What could be the possible cause and what dr is best to consult?

Hi!
A single ankle can swell in gout, rheumatoid arthritis, deep vein thrombosis, trauma, fracture, ligament tear, tendonitis, bursitis or ankle strain. It can also be due to localized infection of the skin above and not be related to the joint.  
X-rays will be negative for all causes except fracture. MRI of the joint may be required to locate the cause. Since swelling comes on prolonged standing it could be deep vein thrombosis or faulty deep vein valves which can be diagnosed by duplex ultrasound or color dopplar. It could also be due to a ligament or tendon injury. It is difficult to comment beyond this at this stage. You will need to follow this up with an orthopedic specialist.
