5 cm complex adnexal mass

Hello:
I am 44 y/o w/ Hx l ovarian cysts that ruptured one year ago and s/p tah 5 years ago. Ever since the tah I have had LLQ pain off and on. Thought it was adhesions.Two weeks ago I have had significant LLQ pain off and on and bloating.Saw my gyn. A mass was detected. The endovaginal and transabdominal probes indicate: "5 cm in diameter l adnexal mass. It contains a large solid component but also contains a cystic component" "There is small amount free fluid in pelvis""The possibility of an ovarain neoplasm such as cystadenocarcinoma cannot be excluded in this case" My gyn called the gyn onc and he veiwed it via stentor and says it could poss. be an endometrioma.I see him this Thurs. for eval. My CBC and ca-125 were negaive. Gr 2 Para 2 (C-sections).
Question: 1. Can you have an endometrioma when you don't have   a uterus, also no hx of endometriosis?  
Question 2: Structurally;how does an endometrioma look similar to a cystadenocarcinoma?
Question 3: What would cause free fluid in my pelvis in my case? I am aware bx is the only diagnostic tool in this situation. Thank you for the service you do. God Bless.
