9cm Right adnexal mass, consisteny w/teratoma-laparotomy

by Fabulous827, May 15, 2007 12:00AM
26yrs, no kids, no history of family ovarian cancers. Seen my gyne and advised her of pains in my lower abdomen.  Scheduled an ultrasound and came back with:

5cm right ovarian mass, presumably dermoid.
3cm left ovarian cyst
fluid in cul de sac

She then had me do an MRI.  results:

Possible right ovarian dermoid mass
Findings: a complex 9cm mass is identified in the location of the right adnexa.  Appearance of mass is complex.  There is a segment of the mass which follows fat signal intensity on all the sequences, is consistent w/teratoma.  Left ovary is slightly prominent and contains multiple cysts w/the largest 2cm.  Uterus unremarkable. The transitional zone of the uterus was not thickened.  There was no fluid in the cul-de-sac.
Impression: 9cm right adnexal mass, mostly consistent w/teratoma.

Dr's interpretation:
I need to have a laparotomy & right ovarian cysteotomy w/possible right oophorectomy.  Is benign, so I have no cancer.

I've done alot of research and know that a cysteotomy usually leaves my ovary intact.  But if further findings suggest otherwise, she'd remove my right ovary.

I'm scheduled to meet w/her later today, and am quite worried if my ovary needs to be removed as I don't have any kids and still want to conceive.  Should I be overly concerned? I'll be scheduling an appt to have the surgery.

Hi there,
It sounds like you are getting good advice and a very good evaluation. You most likely have a dermoid cyst. You need to have that removed. that can usually be done with a cystectomy but occasionally the whole ovary must be removed. We always try to preserve as much ovarian tissue as possible. However, at your age, you should have good fertility even if you only have one ovary
best wishes

Thanks for your advice.  Yes, I will have surgery on 5/24 to have it removed. My Gyne said exactly what you said.  If she could keep my ovary intact she would, but she won't know til she actually gets in there. If not, she said she'd have to remove my ovary but I would still be able to have kids, and very soon (3mths or so) after, I should start as I shouldn't delay trying to have kids in case it comes back, possibly on my left side..so I'm keeping my fingers crossed everything turns out well.

Have one more question, do dermoid cysts become cancerous if not removed? My gyne just said I needed to get it removed.
