Diagnose

Hi Dr. Goodman, I have been in the hospital last July 17, 2011. Based on diagnoses/Findings are Complete abortion, Loculated Right Mass Adnexa and possible Endometrioma. Also i undergo transvaginal sonogram and the impression is:

1.  4.8cm x 2.88cm x 4.15cm (LWT) Right Adnexal Heterogenous Soft Tissue  Lesion with associated loculated fluid collection at its posterior aspect (with internal echoes) as described.  Differentials:
a. Primary Adnexal Pathology
b. Pelvic Inflammatory Disease
c. Ectopic Gestation (?)

2. Very minimal free fluid in the hepatorenal space.

My Question is can you please enlighten me if endometrioma can possibly lead to ovary cancer? Is this really a serious case. I just want to hear your opinion. Thanks.

Hi There,
wow, that is a long time to be in the hospital. I have to assume the you are very sick to need that kind of hospitalization.

it sounds like you have an infection called a tubo-ovarian abscess. Are you pregnant?
An infection in the ovary can be very serious and sometimes if it does not get better with antibiotics, the only way to treat it is to remove the ovary and sometimes both ovaries and the uterus.

It is not associated with ovarian cancer but an untreated pelvic infection can be as dangerous as cancer.

If this is endometriosis, the treatment is also to remove the ovary. If your information is correct and you are 27, the surgeon would work hard to preserve your other ovary and uterus so that your fertility is preserved.

Endometriosis can be associated with ovarian cancer but not at your age - that is seen much later in life.

I hope everything works out
best wishes

Hi there
unfortunately, I cannot give that kind of advice through the internet.Only your doctor who has examined you, looked at the xrays, and evaluated your blood test and cultures can tell which type of antibiotic to take
I wish you the best of luck

Hello Dr. Goodman,

Have a nice day, yes i was pregnant that time which is the reason why i had confined in the hospital due to vaginal bleeding and sad to say it was miscarriage. Honestly i really appreciate your responses regarding my concern. It is quite strange feelings that i have been through this kind of disease. I'm worried about my health, thought i need to be strong and do any consequences to make me feel better. If pelvic infection is untreated, is there any way to stop the infection? Besides may i know how big my endometriosis is compared to? I am hoping also if you can advise me for any antibiotic for me to take? If only i can take only a ride to be in you clinic but i know it is too far from my country. Is it possible that my husband can be infected too? Hoping for your response. Thank you so much.

Sincerely,
Mira
