HCG level up and down, is this germ cell tumor ? Ectopic pregnancy?

My wife is 33 yrs. old. w/ 3 children. 2 weeks ago she went to the ER for pain in her side. A pregancy test indicated positive. The pain was from a cyst...which the Dr. stated will pass. Her initial HCG level was in the 200's (App. 225) Several hours past and a second test indicated HCG's in the low 100's(115). 2 days later a visit to her OBGYN and an addition blood test indicated the level went up again (high 100's) 2 days after HCG level went in 50's. A fifth test indicated back to 100's. Sonogram showed neg. and the Dr. suggested ectopic pregnancy. Further elaborate sonogram performed today shows no baby at all (Tube or uterus). The Dr. is unclear on what to diagnose. Poss. Miscarrage or germ cell tumor! As you can imagine we are seriously concerned! The Dr. stated to wait and continue blood tests for the HCG levels and sonogram. Anyone out there with any helpful comments?

Thank you!  I have my answers now.  I had a tubal pregnancy.  They injected me on Saturday to terminate it.

What happened with your wife?  I have the same thing.  My hcg went from

Hi, I had a germ cell tumor called a immature teratoma 3 years ago.  They originally thought that it was just a large complex cyst but it was the HCG tumor marker that made them believe it was a germ cell tumor.  I believe my numbers were pretty high (higher than your wife's) and did not go up and down.  I had an ultrasound (I am not sure if that is the same as a sonogram...) and it was cleary visible.  My tumor grew to the size of a small watermelon in 3 months so they are not small things to identify.  The immature teratoma I had was pretty rare and there are other types of germ cell tumors that are more common but I am not sure if they can have HCG markers that vary.  After surgery, the HCG levels immediately dropped to normal ranges.  Maybe ask your DR. if an transvaginal ultrasound or CT scan could be done?  Hope everything goes well and they can diagnose her soon.  Best wishes.
