Not sure what to think

Hello, I am a 32 year old woman. I have not experienced menopause. Also, I have never been pregnant.
Early in 2003 I had a pelvic ultrasound (w/trans vag). The ultrasound showed complex bilateral ovarian masses. I had a CA-125, result at 173. Doc suggested surgery but did not refer me to a gyn onco and checked things a month later. The next U/S continued to show a peristent adnexal mass on my right ovary. MY CA-125 remained the same. However, because of other health issues, my surgery was never done.
My pain I've had on that right side has become more problematic over time. A third U/S 7 months after the second one showed the same persistent right mass, but with these findings: the mass representing my right ovary. It is 5.6 * 4.7 * 4.9 cm. It is compartmentalized with at least 4 hypoechoic components separated by thick intervening septations.I also have slight nodularity, and ground-glass echoes withing the cystic components. I still have bilateral masses, the ones on the left more numerous, but more simple appearing. Finally I have a small amount of fluid in the endocervial canal.
I have not had surgery. Numerous doctors have suggested I need surgery, but upon viewing my other history, suddenly don't do anything. I have been to a gyn onco already, but he like the others told me I was too high of a risk for surgery. So, no surgery. I have been in terrible pain and not sure what to do. I cannot do MRI because I also wear a pacemaker. I've been told CT is not good for the pelvis. Can a biopsy be done? I wish I knew the probability of cancer here.  Thanks.

Perhaps it would help if you posted a bit more detail about what your medical history is.  A pacemaker alone would not prohibit surgery.

Just a suggestion.

Yes, my medical history. Unfortunately in the first posting I ran out of room. Anyhow, here it is: Partially treated heart rhythm problems, mild hypertension in my chest, enlarging of my heart, a moderately leaking valve. A prior cardiac arrest, untreated sleep apnea. Bronchospasm and multiple drug allergies. GERD w/large hiatal hernia. Past epilepsy.
Despite all this, I have had about 12 surgeries already. This ovarian procedure would be 13, and so far I've done well with all of them. Only one of my surgeries, however, involved the usage of general anesthesia. That was in 1972. I have never had it again since. If they could only do my abdominal surgery with epidural, as one doctor suggested, I could do fine. But, unfortunately, they only want it done their way, even if it means putting me through excessive risk. Hence, they don't do it at all.

I was in  a similar situation. Ive had reoccuring cysts on both my left and right ovaries. It has been a problem since being put on Coumadin last year for a mechanical mitral heart valve. My GYN/ONC was very leary of subjecting me to another surgery. My last ultrasound showed yeat another cyst on my right ovary this time and she has decided to remove it next week although the surgery and potential for blood loss is a threat. I had a lparascope last August to remove my left ovary and the Dr missed all the ovarian tissue and since then Ive had cysts reoccur on that same left side she removed the ovary. I wish you luck because I know how bad the pain is and my CA levels have also been highly elevated and I have a strong family history of cancer. I wish you all the luck in the world.

A friend, Alicia

Just to respond to your posting. My mass has increased in size. It has now consumed my entire right ovary. As for my surgical risk, several electrophysiologists have considered me a very low risk for surgery. I have had 12 other surgeries with the same body, with the same history. Nothing stopped doctors from operating on me before, until now for some strange reason.

I have found an OB gyn onco who is willing to work with me and my history. One example of how is that I have the option of having epidural anesthesia for my surgery. I have had numerous spinals before without any problems whatsoever.

Numerous doctors have sounded alarms at my ovarian condition being a high risk for cancer. I know I can do very well with surgery with epidural anesthesia. Even though a number of medical providers would rather not do any surgery on me because of my "risk", I believe my current poor quality of life with my being in moderately severe pain every day requiring heavy amounts of painkillers, in addition to my history of other numerous successful surgeries-- the likelihood of an adverse problem occurring with surgery is so small compared to the very real possibility of dying slowly and painfully with cancer.
