U/S~CT~ and CA-125~ Have questions!!

by s-68, Oct 11, 2006 12:00AM
I will try to make this as short as possible.
In August I started getting pain when I would sit. The pain would be sharp in my pelvic area. I had an pelvic and vaginal US on Sept. 11th. Finding were cysts on both ovaries left 1.7cm right 1.4cm. Also  a large amount of fluid present in the cul-de-sac posteriorly and anteriorly and in the adnexa bilaterally. It also said that the fluid seen in the pelvis is of indeterminate etiology.
CT Scan on Sept. 21st. I had the IV contrast but I had a reaction to the oral years ago and they didnt want me to drink it this time. Finding were. There is a cystic appearing area in the left side of the posterior pelvis, measuring 2.5 cm.Adjacent to this on the right there is awhat appears to be free fluid. A complex partially solid mass is seen on the right ovary. It says that an ovarian neoplasm cannot be excluded based on these findings. Follow up ultrasound is recommended. What is ovarian neoplasm?
I had a CA-125 on Sept. 29th, it was 9.
My symtoms are: lower front and back pain on the left, urinating more then normal, weight gain, and bloating (my belly is huge) I have NEVER had weight around my middle!!! I feel like I am walking hunched over because of the back pain.I also have groin and upper thigh pain. My gyn put me on Percocet for the back pain.
I have been dealing with IBS for 15 years and I know that pain well. This isnt it.
I am 38 years old with 5 children. I have had 2 bouts with displasia in 1986 and 1992. Cryo and a leep were done then.
Should I be concerned that this could be serious???

Hi There,
You have developed new pelvic pain that has been persistent since August. You have had two imaging studies that show some fluid in the pelvis and some cysts. It is not possible to make a diagnosis from x-rays. What is important is to make a clinical decision on what is the next step. I would recommend that you talk with your doctor about a diagnostic laparoscopy. It could be that you have a cyst that is bleeding. Usually that causes acute pain and one would see the same ultrasound findings. However, usually with blood from a ruptured  cyst, the pain will get better in a week or two. It is possible that you have a cyst that is twisting around its blood supply and that is causing pain and associated pelvic fluid. It could be that you have a subclinical appendicitis. It could be that you have a tumor on the ovary that is producing fluid.

It is not possible to know which of these diagnoses it is based on the studies. However, pain that has not gone away in two months and that requires narcotics for relief needs to be evaluated.
take care

I too have had IBS for 30 years and at first thought it was that but then started to think no! after long wait and test on bowel found 10cm cyst on right ovary. So im right behind you when you say it's different. You know your body better than anybody. If you feel it's different then push the issue, i wish i had!.

Hi,
I am a thirty-four year old women with no children and one pregnancy.  I have a history of endometriosis and ovarian cyts as well as cervical dysplasia that I had cryo for a few years ago.
My husband and I are being treated for infertility(unexplained, since endometriosis is in remission).  I have been taking Follistim and Ovidrel on and off for 1 year.
I had a cervical dilitation two weeks ago.

Over the last few weeks, I have had major weight gain, bloating, GI issues, and right lower quadrant and sometime excruciating right thigh and leg pain and back pain.  I have a "spare tire" around my mid section.  I have always weighed about 115 lbs. or less so this weight gain is abnormal.  I have had my TSH anf TFT's tested and they are normal.
I have a history of breast cancer in my family at a young age(my mother 39 years old, unilateral mastectomy 18 years and still going survival). I have not taken the fertility drugs in over three months.  Unrelated to these symptoms, I have "flare ups" every so often and during this time I have myalgias, fevers, fatigue and major reflux and chest pain. I saw a Rheumatologist two years ago after a GI doc suggested that i may have an auto-immune disease.  I had a positive ANA and nothing further was done from there.  The Rheumatologist felt that I did not have Lupus or any other disorder.  The symptoms went away for a year or so.

My question?  Is it possible at the young age of thirty-four that I could have ovarian cancer?  What steps should I take from here so my doctor does not think I am crazy?  I have a fertility doctor in Boston and we are due to start an IVF cycle in three weeks.  I am nervous, scared and just annoyed by these symptoms and the lack of diagnoses I have received over the years.
Any suggestions?
Thank you kindly.
Susan
