Ultrasound Report

I am an 18 year old female who has experienced reoccurring cysts throughout the past 2 years. These last 5 months however, I have experienced extreme lower abdominal pain and have been put on narcotics to manage the pain. In addition to extreme lower abdominal pain, i also experience bloating, heaviness in the lower abdomen, lower back pain on the right side as well as pain down the right leg at times.
My last ultrasound taken, the radiologist's first consideration was of Ovarian malignancy with a part fluid-filled part solid-filled cystic mass.
I had a follow up ultrasound on November 23rd(last month) and the ultrasound is as follows... (I was wondering if you could elaborate on what this ultrasound report means.)
PELVIC AND TRANSVAGINAL ULTRASOUND
The uterus is in military position and has a normal size, shape and internal echo pattern. It measures 5.3 x 3.7 cm at the fundus.
The double layer of the endometrial lining measures 1.3cm's in thickness.
The right ovary measures 6.7 x 6.9 x 4.1 cm and contains two cysts:
(1) 4.4 x 4.4 x 3.1 cm complicated cyst
(2) 4.8 x 3.8 x 3.7 cm cyst with echoes
The left ovary measures 1.7 x 1.5 x 1.4 cm.
No pelvic mass or fluid collection is demonstrated.

OPINION: The previously seen right ovarian cyst has increased in size. Endometriosis cannot be excluded.
In total, both ovarian cysts measure up to 9.2 cm. However, if you include the increased size of the ovary, everything in total measures up to 15.9cm.

First of all, what exactly is a complicated cyst? From what I have read about complex cysts, they are more likely to become malignant over time. Secondly, what importance does a cyst with echoes have? Does this mean that there is blood flow throughout? I am really scared because of the last radiologists previous findings. The size of my right ovary seems very big to me, is this abnormal - why would my ovary be so large? I know that I have my age on my side in regards to it being cancer but i can't help but ignore all of these findings. I have surgery booked for next week. Please let me know what this ultrasound report means.

Sorry, another thing I forgot to mention in my post is that this upcoming surgery will be my 3rd surgery due to Ovarian cysts. My first surgery was in January 2006 to remove a 7.7cm septate cyst. Approximately 2 weeks later, I was back in the hospital, only this time for emergency surgery due to a post-op infection as well as 2 more cysts. I was in the hospital for almost 2 weeks. All of these problems began in September of 2005, and since than I have been experiencing ovarian cysts. However, it wasn't until 5 months ago that the pain became horrendous. Another thing I also forgot to mention as far as my symptoms is my change in menstruation since this last cyst. I went 2 months without having a period during my last cycle.
Any opinions or suggestions by anyone including the doctor would be greatly appreciated.

I'm sorry that you have to go through this so young.  I think you are on the right track but make sure that your surgen is a GYN/Oncologist which is very important since they have extra trainning for this field.   Please let us your progress.
